Once I had a patient with an anal wart that was the size of a grape, sticking out of his anus. He admitted that he had had it for months, and I was shocked that he ignored it for so long. Genital warts usually start out as small lesions that have a cauliflower-like appearance. They can spread and grow, but to expand to the size of a grape takes a while. I could not even begin to explain why this patient chose to ignore it for so long. The wart was so large that it had to be removed surgically. What's the point here? Men should not ignore bumps or lesions on the penis, scrotum, or anus. The pathology after his surgery was HPV. Since then, he has routine anal Pap smears to check for recurrence.
